Appendix LL – Slide Request Process
To request slides from the dermatopathology department for grand rounds, tumor board, and other purposes, complete the following Microsoft form. The form automatically notifies the dermatopathology fellows when it’s completed, and all responses are stored in an Excel sheet within their shared folder.
